MHealthy has extended the deadline to complete the MHealthy Rewards Wellness Screening to June 19.
Currently, 51 percent of benefits-eligible faculty and staff have completed the annual health questionnaire and 43 percent have completed a wellness screening. The universitywide goal is 50 percent completion of both.
“Based on feedback we’ve received, the demand is still there so we’ve added a limited number of screening locations and reopened the health questionnaire,” says LaVaughn Palma-Davis, senior director of university health and well-being services. “We are very close to meeting our goal.”
Employees should make an appointment to ensure their screening is 40 minutes or less. Walk-ins also are welcome at all screening locations at the times below; however, those with scheduled appointments will be taken first.
Employees that complete both the health questionnaire and wellness screening receive $100 (before-tax) and an automatic entry into the Rewards Grand Prize Drawings for the opportunity to win one of six prize packages valued up to $750 each.
Wellness screening walk-in hours and locations are:
• Today (June 11) — University Hospital, Ford Lobby/Auditorium, 6-9:30 a.m.
• Tuesday — Michigan Union, Rogel Ballroom, second floor, 8:30-11 a.m. and 1-2:30 p.m.
• Wednesday — Domino’s Farms, Boardroom (by EBA), 8:30-11:30 a.m. and 2-5:30 p.m.
• Wednesday — UM-Dearborn, University Center, Kochoff Hall B & C, 9:30 a.m.-1:30 p.m.
• Thursday — University Hospital, 2C224, 6-10:30 a.m. and 1:30-5:30 p.m.
• Thursday — UM-Flint, Harding Mott University Center, Ontario Room, 9:30 a.m.-1:30 p.m.
• Friday — Taubman Health Science Library, 2955 GAPS Conference Room, 9:30 a.m.-1:30 p.m.
• Saturday — University Hospital, Towsley Lobby, 7:30-11 a.m. and 2-5:30 p.m.
• June 18 — University Hospital, Towsley Lobby, 5:30-9:30 a.m., noon-4:30 p.m. and 7 p.m.-11:45 p.m.
• June 19 — Michigan League, Vandenburg, 2nd floor, 8:30-11 a.m. and 1-3:30 p.m.
