Deadline extended for MHealthy Rewards health questionnaire

Based on faculty and staff feedback, MHealthy has extended the deadline to complete the MHealthy Rewards health questionnaire until May 7.

“We heard from several departments across the university that wanted more time to rally their employees and encourage them to participate,” says LaVaughn Palma-Davis, senior director of University Health & Well-Being Services. “We are excited to see this level of enthusiasm and were more than happy to accommodate by extending the deadline by nearly a month.”

Currently, more than 12,000 benefits-eligible employees, approximately 33 percent, have completed the health questionnaire, the gateway into the MHealthy Rewards program.

Once the questionnaire is completed, the participant can view a personal health status report and health score immediately. Participants also will move on to the second part of the Rewards program, completing two eligible health and well-being activities to earn a $100 before-tax incentive. Employees have until Dec. 3 to complete their two activities.

The deadline extension also allows more time for the university to meet or exceed its goal of 50 percent participation in the health questionnaire.

“This program is designed to support our faculty and staff in meeting their health goals, as well as the university’s goal to become a model community of health where quality of life is enhanced and health related costs are contained,” Palma-Davis says.

To ensure the confidentiality of individual health information, the university has contracted with StayWell® Health Management, a national provider of health improvement services, to administer the health questionnaire.

“MHealthy and the university will not have access to individual health questionnaire results,” Palma-Davis says. “Data received is only in aggregate group form and will provide the university with longitudinal data that contribute to the study of the health of the faculty and staff community.”

To access the health questionnaire, go to www.MHealthy.umich.edu/rewards and click on the “Get Started or Log In” button. Participants will be taken to StayWell’s secure external Web site. A paper health questionnaire can also be requested by calling StayWell at 800-926-5455.
