Award
The Michigan Pharmacists Association has awarded U-M Health System ambulatory group pharmacists an Innovative Practice Award for their work on the Patient Centered Medical Home Team. The award is presented to an individual or team demonstrating an innovative pharmacy practice program resulting in improved patient care or safety, or other advancement. The team members are pharmacists Leslie Shimp, Trisha Wells, Stu Rockafellow, Hae Mi Choe, Heidi Diez, Annie Sy and Tami Remington.
